Urticaria Management in a 23 Year Old Physiotherapy Intern

Ms. R.K. (Patient ID-54599), a 23-year-old physiotherapy intern, visited the Life Force Pimpri branch on 18th May 2024 seeking treatment for chronic urticaria, a condition she had been suffering from since February 2023. She reported recurrent episodes of red blotchy eruptions with intense itching and burning, affecting multiple areas including her limbs, back, chest, abdomen, face, scalp, and genital region.
The episodes occurred predominantly after 9 PM, typically starting with genital itching and gradually spreading across the body. Without medication, symptoms persisted throughout the night. With antihistamines, each episode lasted 4–5 hours. On average, she experienced symptoms three times a week. Her urticaria was aggravated by extreme weather and by consuming peanuts, paneer, and curd.

Past treatment history:

1.Tab. Xyzal M OD, every alternate day (Levocetirizine 5 mg + Montelukast 10 mg), since February 2023

2.Tab. Dazit 5 mg OD, every alternate day (Desloratadine 5 mg), since February 2023

Despite regular use, these medications only provided temporary relief.

Personal history:
She had a mixed diet with small meals at short intervals. She had cravings for sweet and sour foods in general and a dislike for vegetables. She had a low water intake of 1.5–2 L/day with scanty perspiration. She had unsatisfactory hard stools with straining while passing stool and had no urinary or menstrual complaints. She had regular sleep cycles with a preference to sleep on her back or abdomen.

On physical examination, it was noted that she had Grade I pallor with dry skin and a tongue coated at the margins.

She lived in a nuclear family with strong emotional and financial support. Her father is in the construction business, her mother is a homemaker, and her younger brother is pursuing a BBA.

On the mental plane, Ms. R.K. appeared emotionally well-adjusted with strong family support. She described herself as gentle and introverted, easily affected by minor issues, and prone to overthinking. These traits were considered during remedy selection.

Medical History:

History of Grade III hemorrhoid surgery in January 2024

Recurrent cough and coryza since childhood

Family History:

Grandfather: Psoriasis

Mother: Hemorrhoids

Father: Underwent coronary angioplasty

Investigations were advised to her according to case requirements.

After a detailed study of her case, Dr. Rajesh Shah prescribed Carcinocin 30 and other medicines.

Follow-up details:

1st, her first follow-up dated 24th July 2024, she reported an overall 10% improvement. The frequency of episodes reduced to twice a week, each lasting 2–3 hours. Tab. Xyzal was also discontinued.

By the 2nd follow-up dated 12th September 2024, she reported being 30–40% better overall. The intensity and frequency of episodes had reduced.

The patient got her investigations done on 28th July 2024.
Serum IgE—783 IU/mL
She was also advised Vitamin B12 and Vitamin D3 supplements based on low levels.

In her 3rd follow-up by 29th October 2024, she reported being 50–60% better in her complaints.

By the 4th follow-up, 12th December 2024, she reported that her complaint was stable—neither reduced nor worsened—but the dose of antihistamines was reduced. Tab. Dazit 5 mg OD was taken once in 3–4 days, and she was advised to reduce it further to once a week.

In her last follow-up, dated 18th January 2025, she reported that she had not had any episodes of urticaria since the last follow-up.

Conclusion:
Ms. R.K. has shown consistent and significant improvement in her urticaria with homeopathic treatment. From frequent, intense episodes to complete remission of symptoms by January 2025, her progress has been steady. Antihistamine dependency has also reduced significantly, indicating a positive response to the homeopathic approach.
