What is Cervical Spondylitis?

Cervical spondylitis (cervical = neck, spondylitis = inflammation of the vertebrae) occurs due to degeneration (wear and tear associated with aging) of the vertebrae, intervertebral discs, and ligaments in the neck region, known as the cervical spine. In recent times, prolonged use of computers and smartphones has become one of the common causes of cervical spondylitis, even among younger individuals.

The cervical spine begins at the base of the skull and consists of seven vertebrae. These vertebrae are separated by intervertebral discs, which contain a gel-like substance that acts as a cushion and shock absorber. Various muscles and ligaments attached to the vertebrae enable smooth movement of the neck. Nerves emerging from the spinal cord pass through the vertebrae and extend to the neck, shoulders, arms, and upper chest. The cervical spine provides stability to the neck while allowing movements such as bending forward, backward, and sideways. With advancing age, this natural shock-absorbing system gradually degenerates.

In cervical spondylitis, the intervertebral discs become thinner and dehydrated, losing their cushioning ability. The edges of the vertebrae may develop bony outgrowths called osteophytes. In the early stages, some individuals may experience no symptoms, while others may develop neck, shoulder, or arm pain and stiffness. These symptoms arise from inflammation of the surrounding soft tissues and ligaments, as well as from the formation of osteophytes. Over time, these changes can compress nearby nerve roots, leading to pain, tingling, numbness, or weakness in the upper limbs.

As degeneration progresses, narrowing of the spinal canal or the openings through which spinal nerves exit may occur. This condition is known as cervical spinal stenosis or spinal canal stenosis, and it can further contribute to nerve-related symptoms.

Risk factors

A history of neck injury, poor posture, and occupations involving repetitive neck strain also elevate risk. Genetics, a sedentary lifestyle, weight gain, and smoking may further accelerate degeneration of the cervical spine, making symptoms more likely to appear.

Key Symptoms

Cervical spondylitis can cause neck pain radiating to the shoulders, arms, or hands, often worsening with movement or prolonged sitting. Stiffness after inactivity is common and may limit neck mobility. Some patients experience muscle weakness while lifting or gripping objects. Nerve compression can lead to shooting pain, tingling, or numbness. In advanced cases, spinal canal narrowing may cause poor coordination, hand weakness, or muscle spasms.

Underlying Changes

Cervical spondylitis develops from age-related wear and tear where spinal discs lose elasticity and cushioning, causing vertebrae to rub together. As degeneration progresses, discs may bulge or herniate, ligaments stiffen, and bony spurs form. These changes can compress nearby nerves, leading to pain, numbness, or weakness, and in some cases may narrow the spinal canal, affecting movement and sensation.

Accurate diagnosis

Diagnosis of cervical spondylitis involves a detailed clinical evaluation, including medical history, physical examination, and assessment of neck movements, reflexes, muscle strength, and posture. Imaging studies such as X-rays, CT scans, MRI, or myelography are used to assess disc degeneration and nerve compression.

In some cases, additional nerve studies, such as electromyography (EMG) and nerve conduction studies (NCS), may be required for further evaluation.

Conventional treatment

Conventional treatment for cervical spondylitis mainly focuses on reducing pain, stiffness, and improving neck mobility through medications.
