A 52-year-old male mechanical engineer from Udaipur presented with complaints of persistent papular eruptions confined exclusively to the hairy parts of the body. The eruptions initially started one year ago and were observed on the shoulders, hands, back, buttocks, thighs, and legs. They appeared as tiny red lesions accompanied by redness and dryness, gradually increasing in intensity and coverage over the course of the year. Notably, there was no involvement of the genitals. These lesions were mildly tender and often followed a cyclic course -appearing and then fading ,leaving behind bald patches and black hyperpigmented marks.
The chief presenting symptom was intense itching , which would increase with the appearance of the lesions and persisted even in the absence of active eruptions. The patient described the itching as moderate to severe, and despite trying conventional treatment, including two months of steroid therapy, the itching persisted and new eruptions continued to appear. He discontinued steroids two days before the initial homeopathic consultation.The condition was continuous and gradually progressing, with no complete remission noted in the past year.
The patient also complaints of dryness of the skin and the persistant hyperpigmented spots even after the eruptions subsided.
Past Medical History
Chickenpox in childhood and a dog bite for which he had received 11 anti-rabies injections. There was no significant past dermatological illness noted before the current complaint.
Family History
Has a strong genetic predisposition to chronic illnesses father had hypertension and diabetes and passed away due to lung pleuritis; mother was diabetic and hypertensive and succumbed to ischemic heart disease. An elder sister was also hypertensive.
Physical Generals
The patient was thermally hot, had a mixed diet with average food intake, a strong craving for spicy food, and an aversion to sour items. He consumed 2–3 liters of water per day, had average perspiration, satisfactory bowel movements (1–2 times/day), and normal sexual functioning. His sleep was sound with no disturbing dreams.
Mental Generals
A person of expressive temperament who shows anger by shouting but often experienced guilt afterward. He described his life as happy and felt well-supported by his family. He lived with his homemaker wife and two children one pursuing a diploma in mechanical engineering and another completing an MCA.

Follow-Ups
On 28th December 2023
The first follow-up showed initial good results. There were no new eruptions, and older ones had reduced noticeably. The patient reported relief in itching. He had completely stopped steroid use .
On 29th February 2024
Improvement continued steadily. No new eruptions had developed, and existing ones were shrinking. Hyperpigmentation remained, and itching was still present but slightly reduced in frequency. The patient had now fully stopped all conventional medicines and was only on homeopathic treatment.
On 8th May 2024
There was a further reduction in both the size and intensity of older lesions. Hyperpigmentation was visibly lighter, but persistent, continuous itching remained the major distressing symptom. The patient had started taking desloratadine 5 mg occasionally for temporary relief but found homeopathy helping in the overall progress.
On 18th June 2024
The patient reported consistent improvement in skin condition with no fresh eruptions. Old lesions on the legs, thighs, and hands continued to fade. However, itching was severe, especially post-scratching. Small papules occasionally appeared but would self-resolve within 20–22 hours. Hyperpigmented marks were slowly lightening.
On 6th August 2024
The patient presented with 70-80% improvement. Old eruptions on the legs and hands had significantly improved. Itching had decreased substantially, and he no longer required antihistamines. There were no new lesions since the last visit, and only dark spots from past eruptions were seen. He was not on any conventional medicine and was highly satisfied with the homeopathic approach.
Conclusion
This case illustrates the effectiveness of homeopathic treatment in managing Lichen Planopilaris. Over several months, the patient experienced complete cessation of new eruptions, visible healing of older lesions, and gradual resolution of intense itching.
