Lichen Planus Pigmentosa is a chronic autoimmune disorder where the grayish brown marks on the face and neck or trunk and limbs are without any inflammation; it is just the hyperpigmentation of skin associated with dryness and occasional itching.
Mrs. RM had hyperpigmented patches on the forehead, cheeks, nose, and neck, with no dryness or itching associated, since June 2023. She was a business analyst, so this hyperpigmentation was bothering her as she had to meet people and give public appearances, and it affected her confidence and her performance at the workplace. So when she was consulted for her complaint and the scope and treatment of her disease were explained, she decided to start the treatment because she was aware that homeopathy would help her to control her disease. She had tried a lot of steroidal ointments like tacrolimus and other cosmetic creams, but they were giving her temporary relief, and patches would darken again as she stopped using them.
She shared her biopsy result done on August 2023, which confirmed LPP
-The epidermis shows features of orthokertosis, spongiosis, and basal cell vacuolation, with increased melanocytes in the basal and suprabasal layers
-hypergranulosis and significant lichenoid dermatitis are not seen epidermal and dermal interface shows mild fibrosis dermis shows mild perivascular lymphocytic infiltrate melanophages are seen in the superficial dermis.
A detailed case history was taken on call.
She also had an underactive thyroid since 2006, for which she was on a thyroid supplement and was stable with Tablet. Thyronorm 75 mcg
Physical generals:
She was a vegetarian person with a liking for chocolates and a dislike of spicy and sour food, with an average thirst of 2-3 lts/day, moderate perspiration, and no complaints with urine and stool. Her menses were regular, and she had a sound sleep.
Mental generals:
She is a very warm person, talking in a very sweet and humble way. She is easy to get along with, expressive, ambitious, confident, and likes to talk and meet new people.
Family history: Her mother and aunt also have an underactive thyroid, and her father has depression.
Past history: She had suffered from sinusitis, typhoid, and chickenpox in the past.
After reviewing the case and the pictures sent by the patient, Dr. Shah prescribed SEPIA along with dietary and lifestyle change recommendations.
Follow-ups:
First follow-up:
On 6 Nov 2023, the hyperpigmentation appeared to be stable with no new areas being affected, comparing the photos and the follow-up taken on the call.
Second follow-up:
On 6.2.2024, pigmentation was improving in the areas of the cheek and neck, while hyperpigmentation on the nose and forehead remained stable. She was happy with the improvement and appreciated the homeopathic action.
Third follow-up:
On 6.4.24, she noted a 50% improvement in pigmentation, and the skin dryness was improving; no new areas were affected; the patches were now becoming lighter and lighter with each follow-up.
Fourth follow-up
On 3.6.24, a slight increase and flare-up of patches were noted as she had traveled and was exposed to the sun. She used ointment tacrolimus along with homeopathic medication for 10 days and then stopped it.
In the coming follow-ups, she improved in pigmentation, and a visible difference was seen after comparing her before-and-after photos. She was extremely happy seeing herself in the mirror again.
She was completely better and so stopped treatment for LPP but started treatment for vertigo, which she has been facing for a few months.
