What is Ankylosing Spondylitis?

Ankylosing spondylitis (AS) is a chronic inflammatory and degenerative joint disorder. It primarily affects the spine and the sacroiliac joints that connect the spine to the pelvis. Inflammation of the joints, commonly referred to as arthritis, gradually leads to pain, stiffness, and reduced spinal flexibility.

As the disease progresses, spinal joints may fuse, resulting in persistent pain and restricted movement. This advanced stage is often called “bamboo spine.” When rib joints are involved, chest expansion may be reduced, causing breathing difficulty. In some cases, Ankylosing Spondylitis may also affect the eyes, heart, lungs, or, rarely, the kidneys.

Genetic factors play a significant role. Many patients carry the HLA-B27 gene, although not everyone with this gene develops the disease. Early diagnosis and structured management can help slow disease progression and reduce long-term complications.

Pathogenesis of Ankylosing Spondylitis

Ankylosing spondylitis develops due to a complex interaction of genetic (HLA-B27), immune, familial, stress-related, and environmental factors. The core pathology involves enthesitis—inflammation at sites where tendons and ligaments attach to bone—particularly affecting spinal structures.

Persistent inflammation leads to cartilage damage, followed by abnormal bone formation (syndesmophytes), resulting in fusion of vertebrae and sacroiliac joints.

Causes of Ankylosing Spondylitis

Ankylosing spondylitis is an autoimmune condition where the immune system mistakenly attacks joint tissues. Genetics plays a major role, with nearly 90% of affected individuals carrying the HLA-B27 gene, though its presence alone does not cause the disease.

The condition usually begins between 20 and 30 years of age and is more common in men. Long-term stress, infections, and environmental triggers may activate immune responses, leading to chronic joint inflammation.

Early Symptoms of Ankylosing Spondylitis

• Persistent lower back or hip pain
• Morning stiffness improving with movement
• Fatigue and discomfort after rest
• Mild neck or lower back restriction

 

Progressive Ankylosing Spondylitis Symptoms

• Severe spinal stiffness and reduced mobility
• Neck pain and postural changes
• Fusion of spinal joints
• Chest tightness affecting breathing
• Pain in the hips, knees, ankles, or shoulders
• Enthesitis causing heel or foot pain (Achilles tendon, plantar fascia)

 

Complications

Ankylosing spondylitis can affect organs beyond the joints. The most common complication is uveitis (iritis), presenting with eye pain, redness, light sensitivity, and blurred vision. If untreated, it may impair vision.

Rarely, a long-standing disease may cause secondary amyloidosis, affecting kidney function and leading to fatigue, protein loss in urine, and progressive renal damage.

Diagnosis

• Clinical history (chronic inflammatory back pain, morning stiffness, uveitis, family history)
• Physical examination (restricted spinal and chest movement)
• Blood tests (HLA-B27, ESR, CRP; negative RA factor and ANA)
